Learning disabilities liaison team
Our learning disabilities liaison team helps us to provide care and treatment that meets your needs. This includes making any reasonable adjustments.
The team will also make sure you understand your care and treatment plan.
We can also offer you longer appointment times and encourage you to bring any tools that you use (e.g. communication boards).

My hospital passport
We use a ‘hospital passport’ to help you or your carer to tell us important information about you and your needs.
If you would like to discuss any communication or other needs that you may need additional support for, please contact the ward or department (in person, by email or telephone) using the contact details on your appointment or admissions letter.
