The Community Neurological Rehabilitation Team (Brighton & Hove) is part of Sussex Community NHS Foundation Trust.
We are a multidisciplinary neuro rehabilitation team for people aged 16 and over who have specific long term neurological conditions.
We also provide an Early Supported Discharge service for patients who have had a stroke. The service is always provided in peoples own homes or in care homes.
