Throughout your life your personal details may change for one reason or another. It is very important that the hospital holds an accurate record of these for you.
Please ensure you notify your GP of any changes to your personal details or, if urgent, please contact the hospital before your next attendance.
If you already have an appointment, then you can call the number on your appointment letter and speak to one of the booking team who can update your records for you over the phone. You can also ask your consultants secretary to do this for you.
Alternatively, you can tell us in person when you come for your next appointment.
