Spinal Trauma Referral 

This pro-forma is for referral of a spinal injury to the BSUH Spinal Surgeons only (Mr Cass and Mr Karmani).  It is only to be used in relation to referrals made to them.

The pro-forma should be filled in as completely and accurately as possible.  Failure to complete may mean return of the form.

The mandatory ASIA exam & score should be performed by the most senior and experienced doctor available.

The completed pro-forma should be forwarded after verbal discussion with one of the following:

Monday-Friday 8.00am - 5.00pm: Trauma Co-Ordinator (Bleep 8165 @ Royal Sussex County Hospital)

Weekends and 5.00pm - 8.00am: On-Call Orthopaedic Registrar at RSCH

The fax/email address for forwarding will be given by the above.

Do NOT forward directly to Mr Cass or Mr Karmani unless instructed to do so.

Please note that receipt/acceptance of the pro-forma is NOT an acceptance of the patients, nor does it constitute a transfer of care.
